About Living Well with Persistent Pain Program
The Living Well With Pain Program is an 8-week program that aims to provide participants with the knowledge and skills required to manage their pain at home and within the community. This program is run by PHM for individuals living with persistent pain and focuses on providing support and care to individuals who experience chronic or long-term pain. The goal of a PHM program for persistent pain is to help individuals manage their pain and improve their quality of life.
Adults over 18 years who have experienced consistent pain that has lasted more than 3-6 months are eligible for this program.
The program utilises local health care resources like Physiotherapists, Exercise Physiologists and Allied Heath Care Assistants at Adelaide Active Rehab & Therapy to support patients with their pain management in the community.
As part of this program Adelaide Active and Rehab provide tailored physiotherapy exercise programs for clients.
